Cancellation Policy

Confidentiality All information disclosed within sessions and the written records pertaining to those sessions are confidential and may not be revealed to anyone without your written permission except where disclosure is required by law. Clinicians frequently discuss the details of certain cases in supervision with other clinicians, and information shared in that environment will also remain confidential. Soulliaa Foley and SoulRaez partners will not release records to any outside party unless authorized to do so. Insurance You understand that a "super bill" may be prepared for you to seek reimbursement for services through health insurance. You acknowledge that the submission of these forms is your responsibility and reimbursement from insurance is not a guarantee that can be made by SoulRaez partners Communication You are consenting to receive communications from Allia “Soulliaa” Foley and SoulRaez partners via email, text, and phone call. Unless otherwise indicated by an agreement, clinicians and staff are available during business hours between 8am - 5pm. Staff are not to be contacted in cases of emergency - in the event that you are at serious risk of harming yourself or someone else, please contact 9-1-1. In the event of a mental health crisis, including if you are experiencing suicidal ideation with the intent to act on a plan, or ideation to physically harm another person with intent to act on a plan, please contact the 24-hour Harris County Mental Health Crisis line: 713-970-7000 and press 1. If you are prone to experiencing different forms of acute mental health crisis, please be sure to communicate this with your clinician so a safety plan can be established for how these circumstances will be handled by your clinician. Audio or Video Unless otherwise agreed to by all parties beforehand, there shall be no audio or video recording of therapy sessions, phone calls, or any other services provided Cancellation Soulliaa requires 48 hours notice for any cancellations or changes to your appointment. Clients who provide less than 48 hours notice will be charged 50% of their appointment fee at the first late cancellation, and 100% of the service fee for subsequent no-shows or late cancellations. Fees charged for cancellations and no-shows are at the discretion of the clinician in the case of exigent circumstances.
